Lodi Municipal Service Center Return to Compliance Statement of Facts CCR 2711 UST operating permit application for facility and flank information not submitted or current. The second 87 tank vent piping was corrected on CERS from single wall to none. The diesel tank vent piping was corrected from no vent piping to steel piping on CERS. CCR 2637(e) Failed to submit secondary containment test report within 30 days. Secondary containment testing was performed on October 24, 2017 and a test report was submitted on December 13th 2017 one month late. The owner operator will make sure in the future that all test reports will be submitted within 30 days. CCR 2715(a) Failed to submit to EHD notification of designated operator identification or change within 30 days. Updated designated operator identification was uploaded to CERS on April 20`h 2018 with the incorrect attachment, the correct attachment was uploaded to CERS on 9/17/2018. The owner operator will make sure that the correct attachments will be uploaded to CERS in the future. CCR 2712 (b) Failure to maintain monitoring, maintenance, and follow up action records. The Maintenance and monitoring records for the last three years were out of order in the binder on site and now placed in the proper position in the binder. The secondary containment test results and repair documents have been placed in the onsite binder for future reviews. Copies of all above documents attached along with the Return to Compliance Statement of Facts. CCR 2715(c) Designated operator did not inspect all required items anti/or the inspection reports not completed. The Designated Operator has corrected the dates in all the monthly inspection forms for the secondary containment test completed on October 242017, CCR 2715(f)(2) designated operator employee training not performed or log no kept. Employee training was given on 6/1/18 and a training log was filled out and placed in the onsite binder at the time of the monitoring certification test.
