Ham Lane, Lodi, CA, 95240 SR0078415 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to install new DPM URBK series penetrations on the product lines in the Under <br /> Dispenser Containment (UDC) of dispenser 4/5 and install a new DPM URBK series penetration in the <br /> 2,000 gallon, 87 product STP sump of the Underground Storage Tank (UST) system at the above noted <br /> address. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. A 48-hour notice is required for inspection appointments. Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> • Sensor functionality: Functionality of the monitoring system for UDC 4/5 sensor must be <br /> verified to ensure that monitoring has been restored to meet the requirements of Title <br /> 23 California Code of Regulations and the Health & Safety Code. <br /> • A 24-hour hydrostatic test (or equivalent) on UDC sump 4/5 and on the 87 product STP <br /> sump. <br /> • Visual inspection of piping within dispenser 4/5. <br /> 4. If concrete will be broken near the tank a secondary containment test must be performed on the <br /> affected USTs once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be <br /> issued to the installation company), or a Tank Tester's license issued to the individual by <br /> the State Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> 1868 E.
