Failure to submit Underground Storage Tank Permit Application -Form A and Underground <br />Storage Tank Permit Application -Form B, in violation of California Code of Regulation, <br />Title 22, section 2711(b); <br />b. Failure to maintain evidence of financial responsibility for taking corrective action and from <br />compensating third parities for bodily injury and property damage caused by a release from <br />an underground storage tank system, in violation of Health and Safety Code section <br />25292.2(a); <br />C. Failure to test secondary containment by January 1, 2003, in violation of Title 22 of the <br />California Code of Regulation section 2637(a); <br />d. Failure to develop a monitoring and response plan, in violation of California Code of <br />Regulation 2712(i); <br />e. Failure to have maintenance and monitoring records, in violation of California Code of <br />Regulation 2712(b); <br />f. Failure to have a designated operator to perform monthly inspections, in violation of <br />California Code of Regulation 2715(c); <br />g. Failure to maintain a copy of the designated operator monthly inspections, in violation of <br />California Code of Regulation 2715(e); <br />h. Failure to have facility employees trained by the designated operator by July 1, 2005, in <br />violation of California Code of Regulation 2715(f); <br />Failed to notify the local agency when required to do so, including failure to notify the local <br />agency of any change in the designated operator no later than 30 days after the change, in <br />violation of Health and Safety Code section 25299(c). <br />Failure to remove water from the secondary containment, including fuel/water mixture not <br />removed from spill buckets, in violation of Health and Safety Code section 25291(e). <br />HAZARDOUS WASTE VIOLATIONS <br />a. Failure to post emergency coordinator information next to the telephone, in violation of <br />California Code of Regulations section 66262.34(d)(2). <br />
