EI Dorado St., Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 703. At the time of inspection, it could not be demonstrated (not necessarily documented) that <br /> employees who handle hazardous waste were properly trained. According to the employees on site, <br /> hazardous training is conducted in Walnut Creek every three years. The generator must ensure that all <br /> employees who handle hazardous waste are thoroughly familiar with proper waste handling and <br /> emergency procedures. Immediately provide training to all employees who handle hazardous waste and <br /> submit a copy of the training records to the EHD by February 4, 2013. <br /> 804. Used fluorescent bulbs were observed stored open and uncontainerized on a shelf in the shed next <br /> to the office. Universal waste lamps shall be contained in a closed container or package that is <br /> structurally sound, adequate to prevent breakage, and compatible with the contents of the lamp. <br /> Immediately containerize all universal waste fluorescent lamps/bulbs in accordance with Title 22 <br /> regulations. <br /> 808. Used fluorescent bulbs in the shed next to the office and a bucket of universal waste batteries in <br /> the shop were observed without labeling. A generator shall label or mark universal waste with the words <br /> 'Universal Waste' and the type of waste (batteries, mercury containing equipment, lamps/bulbs, <br /> electronic devices, CRTs, CRT glass, aerosol cans). Immediately label these wastes and ensure that all <br /> universal wastes are labeled with the required information. <br /> 812. Copies of universal waste disposal records were not found on site. A generator of universal waste <br /> shall keep a record of each shipment of universal waste on site for at least three years. The record may <br /> take the form of a log, invoice, manifest, bill of lading, or other shipping document, and shall include the <br /> following: <br /> - Name & address of destination facility <br /> - Quantity & type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by February 4, 2013 <br /> along with a statement documenting the corrective actions that have been or will be taken for each <br /> violation and any supporting paperwork. <br /> Provided: Modified Contingency Plan template, Schedule of FREE classes, Sample hazardous waste <br /> label, Hazardous waste labels, Small Business Household Hazardous Waste Facility brochure, CERS <br /> Lead User Authorization form, CERS HMMP flyer, Return to Compliance Certification form <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT AMY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Redel ed By: Title: <br /> Stacy Rivera (209) 468-3440 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
