The following is an itemized list of hazardous waste violations that have not been addressed for FAST N ESY as of September01, 2015. Open violations from June 11, 2015 inspection Violation #105 - Failed to train employees on waste handling and emergency procedures. At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle hazardous waste were properly trained. Ms. Sukhpreet Kaur, who was the only employee onsite at the time of inspection, stated that she was not sure what she should do with the waste absorbent after it was used for spill clean up. The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste handling and emergency procedures. Immediately provide training to all employees who handle hazardous waste and submit a copy of the training records to the EHD. This is a repeat violation and was also listed on the 201 inspection report. Violation #116 - Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 6-5-2014 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 7-5-14; RTC form only was received on 9-29-2014. This information has not been received resulting in a non-compliant status for this facility. A operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Violation #304 - Failed to maintain adequate aisle space for unobstructed response to emergencies. One 55 -gallon container, which was not empty and had a hazardous waste label, was blocked by several heavy boxes. Adequate aisle space shall be maintained to allow the unobstructed movement of personnel, fire protection, spill control equipment, and decontamination equipment to all areas of facility operation in an emergency. Violation #404 - Failed to inspect hazardous waste storage areas at least weekly. Hazardous waste containers are not inspected weekly. A peeling hazardous waste label on one of the 55 -gallon containers and another blocked 55 -gallon hazardous waste container were noted. Hazardous waste container storac and transfer areas shall be inspected weekly for leaking containers and for deterioration of containers and the containment system caused by corrosion and other factors. Immediately begin conducting the necessary weekly inspections for all hazardous waste containers at this facility. Violation #601 - Stored hazardous waste onsite greater than 180 days. One 55 -gallon black (non -empty) container had an accumulation start date of 2-24-14 and one had a peeling unfilled hazardous waste label, both these containers were located behind the store inside fenced in storage area . Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site a any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. Violation #605 - Failed to completely label containers or tanks of hazardous waste. One 55-galon (non -empty) black container had an unfilled and peeling hazardous waste label. All hazardous waste containers shall be marked with the following information: - the words "Hazardous Waste" - name and address of generator - hazardous properties - physical state - composition (contents) - accumulation start date Immediately label these containers and ensure that all containers are marked with all the required information. SincE it cannot be determined how long this container has been on site, immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD.
