YOSEMITE AVE <br />MANTECA, CA 95336 <br />In late 1?83, the Governor signed into law A620 3 and 1362. These require <br />�"1_ ^'�' "th___ Biller _��irr_ <br />the inventory, inspection and permitting of all underground storage tanks that <br />contain hazardous_. materials. The San Joaquin Local Health District, Division <br />of Environmental Health -was designated as the enforcement agency for the <br />cities and unincorporated areas within Son Joaquin County. State law provides <br />for a fee system to cover the cost of implementing this state mand ted <br />program. <br />Local fees (see attached Free Schedule), for these required inspectional <br />services? will be billed on _. yearly basis. The facilities Permit to Operat_, <br />will be issued for a five year period. In addition to the yearly <br />inspectional fee, this statement will also include a $56/tank State surcharge <br />free. The State surcharge fee per tank will be charged ever= five years or <br />whenever the facilities permit is renewed or amended. All State surcharge <br />fees .,ill be transmitted by the Health District to the California Water <br />Resources Control Board. <br />To apply for a facilities Permit to Operate; complete the attached Fee <br />Worksheet and submit the appropriate fees. Fees are due and payable February <br />3, 1986. Fees received after March 6, 1986 are subject to 1001 penalty. <br />In order to answer any question= regarding, this Underground Storage Tank <br />program, the San Joaquin Local Health Distict Division of Environmental Health <br />has.s.cheduled the following information workshops; <br />1. Thursday, January 30, 081, 10 AM <br />2. Monday, February 3, 1936, E PM <br />3. Tuesday, February 11, 1936, 2 Pini <br />All of the above informational workshops will be conducted at the San Joaquin <br />Local Health District Board Room, 150.1 E. Hazelton Ave., Stockton, CA.
