The DO noted that the alarm occurred during a delivery, but did <br /> explain how the alarm was related to the delivery. <br /> '1 <br /> On the 1/18/2010 DO monthly inspection report,the DO documented and addressed a low liquid alarm from <br /> 12/16/2016, but did not indicate that the alarm was L6. <br /> On the 3/14/2017 DO monthly inspection report,the DO documented three L8 annular space high liquid alarms and <br /> notified the owner to contact Donlee Pump Company to look at the sensor. No work order addressing the L8 sensor <br /> was found on site. On the 4/03/2016 DO monthly inspection report, the DO documented the L8 sensor out alarm from <br /> 3/16/2017, and that the alarm cleared on its own.The DO did not indicate that there was also a high liquid alarm for <br /> the L8 sensor on 3/16/2017, as seen on the alarm history. Immediately submit the work order to the EHD or submit a <br /> statement explaining why the alarm was not looked at by a contractor. <br /> Ensure that designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> This is a repeat violation, Class II. <br /> 302 HSC 25291(b) UST system incapable of detecting leak from primary into secondary containment(pre-Jul 2003). <br /> The boot in the diesel submersible turbine pump sump was clamped tight and the Schrader valve was not facing <br /> toward the bottom of the sump. Any potential release from the primary containment to the secondary containment in <br /> the fuel piping does not flow into the sump. All secondary containment systems shall be sloped so that all releases <br /> will flow to a collection sump located at the low point of the underground piping. The boot should be loose or pulled <br /> back so that releases into the product piping secondary can reach the closest sump and the UST monitoring system <br /> will be able to detect a leak at the earliest opportunity. Immediately contact a properly licensed, trained and certified <br /> contractor to ensure that any releases in the product piping flows into the facility turbine sump. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91-product submersible turbine pump sump 208 sensor was located a few inches above the lowest point of the <br /> saddle of the tank and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be <br /> maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest <br /> point of the sump by the service technician.
