Financial responsibility documents for 2013 year have not been submitted to the EHD. Current <br />financial responsibility documents are required to be submitted annually. Complete and submit a copy <br />of the financial responsibility to EHD or submit copy online to CERS. <br />305. The center Bravo float in Dispenser 1/2 sump failed to stop the flow of product at the dispenser <br />when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a <br />properly licensed, trained, and certified contractor replace the failed component with a functional <br />component (LG 113-x listed, if applicable). <br />-Corrected on site. Technician removed liquid from Bravo float, reinstalled and retested- passed. <br />Submit to SJC EHD within 30 days (06/24/13): <br />1. one copy of the completed "Return to Compliance Certification" form <br />2. a written corrective action statement for each of the above violations <br />3. copy of financial responsibility to EHD or online to CERS <br />Notes: <br />-Facility has not submitted CUPA data online (Business Plans, Hazardous Material, Underground <br />Storage Tanks, Emergency Response, Training,etc) to California Electronic Reporting System <br />http://cers.calepa.ca.gov Facility CERS ID is 10181551. <br />Please contact Thuy Tran, ph 468-8257, to make an appointment for assistance on online reporting <br />-Site officially open in Sept 2012 after the construction/ repairs completed. <br />-Programing setup change. When performed initial sensor out, the alarm printout 'fuel alarm' instead of <br />sensor out'. Reviewing monitoring system liquid level setup, liquid sensors incorrectly programmed to be <br />normally closed'. Technician reversely installed the magnetic float sensors in annulars and STPs, then <br />reprogrammed liquid sensor set up from 'normally closed' to 'tri-state (single float)'.
