The designated operator <br /> listed April 5, 2016 as the test dates for the monitoring system certification and spill container. The monitoring <br /> system certification and spill container testing with an inspector present were completed on May 3, 2016. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system <br /> have been completed and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 302 HSC 25291(b) UST system incapable of detecting leak from primary into secondary containment (pre-Jul 2003). <br /> The diesel annular sensor failed to activate an audible and visual alarm when tested and, therefore, the tank is not <br /> monitored. The interstitial space shall be monitored with a monitoring system connected to an audible and visual <br /> alarm system. The sensor was replaced and retested during the inspection. A"UST Retrofit Verification with <br /> Inspector Already Onsite"form has been completed and provided to the operator and will be sent by email to the <br /> contractor. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 314 CCR 2631(a) Primary containment not tight (pre-Jul 2003). <br /> Oily debris was found below the shear valve in under dispenser containments for dispensers 1-2, 5-6, and 7-8, <br /> indicating a possible leak in the primary piping. All primary containment for the UST system must be tight. <br /> Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component under <br /> permit and inspection of the EHD. <br /> This is a minor violation. <br /> 319 HSC 25291(e) Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Approximately one cup of liquid was observed in the diesel submersible turbine pump (STP) sump. If water could <br /> enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. The <br /> service technician removed the liquid from the diesel STP sump. Ensure that all sumps and annular spaces are <br /> maintained free of liquid. <br /> This was corrected on site. <br /> This is a minor violation. <br /> Page 5 of 6 <br />
