Records submitted by the facility indicate the manifest was submitted to Betty Ho and Michelle <br /> Henry via email; both are inspectors for SJCEHD and do not work for DTSC. The generator manifest copy shall be <br /> submitted to DTSC for every shipment on a manifest when California is either the generator state or the destination or <br /> consignment state. The generator manifest copy shall be mailed to: <br /> DTSC Generator Manifests <br /> P.O. Box 400 <br /> Sacramento, CA 95812-0400 <br /> Immediately mail a copy of the manifest listed and ensure that copies of all generator manifests are sent to DTSC <br /> within 30 days of shipping hazardous waste. <br /> This is a repeat violation, Class ll. <br /> 117 HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 9/14/17 and an inspection report was issued identifying information to be submitted to <br /> bring this site into compliance. This information was required to be submitted by 10/14/17. This information has not <br /> been fully received resulting in a non-compliant status for this facility. The outstanding violations include. <br /> -Facility not maintained to minimize releases of hazardous waste (fuel filters not contained within secondary <br /> containment) <br /> An operator that receives an inspection report shall have 30 days to submit a written response that includes a <br /> statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a <br /> written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an <br /> inspection report. <br /> This is a repeat violation, Class II. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> The fuel filters in UDCs 1/2, 3/4, 5/6, and 7/8 are not contained within the secondary containment(under dispenser <br /> containments) for the dispenser piping. Facilities shall be maintained and operated to minimize the possibility of a fire, <br /> explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the <br /> environment. Immediately Immediately make all necessary alterations to prevent leaks from the fuel filters to the <br /> environment. <br /> This is a repeat violation, Class II. <br /> 601 CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br /> A 5 gallon white plastic bucket of waste absorbent was observed in the hazardous waste storage area with no <br /> accumulation start date. The most recent disposal record on site is dated October 2016. Facilities who generate less <br /> than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store <br /> waste on site up to 180 days. Since it cannot be determined how long the hazardous waste has been on site, <br /> immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest.
