Victor Rd. Lodi, CA 95240 Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br /> bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br /> EHD by 1-19-12. <br /> 405. A black 55 gallon drum of hazardous waste liquid in the storage room is not being inspected <br /> weekly. Nor is there documentation on site to show how often these containers of hazardous waste are <br /> being inspected. Mr. Pahwa stated that this drum is checked on monthly. Containers of hazardous waste <br /> must be inspected weekly. Correct immediately by implementing weekly inspections of any facility <br /> hazardous waste containers. <br /> 605. A black 55 gallon drum of hazardous waste liquid in the storage room has a hazardous waste label <br /> with faded ink that is now illegible. All hazardous waste containers shall be marked with the following <br /> information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label this container and ensure that all hazardous waste containers are marked with all the <br /> required information. <br /> Note: Accumulation time limits for a conditionally exempt small quantity hazardous waste generator were <br /> explained to Mr. Pahwa. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by 2-19-12. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453 Ij�( l�►^I / <br /> EHD 23-02-003 Rev 07/12/11 CONTINUATION FORM <br />
