Copies of hazardous waste disposal records for test water for the last three years were not <br /> found on site. The only disposal record on site for spent absorbent for the last three years was dated <br /> March 7, 2014. According to Ms. Mayra Reyes, the store manager, the waste water was recently <br /> removed by a hazardous waste hauler. Hazardous waste generators shall retain copies of all manifests <br /> signed off by the disposal facility and all receipts used in a consolidated manifesting procedure on site for <br /> three years and have them readily available for review. Immediately locate a copy of all manifests and <br /> receipts for the last three years, maintain them on site, and submit copies to the EHD by May 3, 2014. <br /> 605. A 5 gallon plastic bucket labeled "used absorbent" was observed in the store room in the facility <br /> next to the new absorbent. No other labeling was observed on the bucket. According to Ms. Mayra <br /> Reyes, this is the waste absorbent. All hazardous waste containers shall be marked with the following <br /> information: <br /> 1. the words "Hazardous Waste" 4. physical state (liquid, solid) <br /> 2. name and address of generator 5. composition (contents) <br /> 3. hazardous properties (flammable, corrosive, reactive, toxic) 6. accumulation start date <br /> Immediately label this container and ensure that all containers are marked with all the required <br /> information. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br /> Inspector: Received By.
