The permit has been approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval prior <br /> to commencing work . All new addenda supersede the previous ones . <br /> 3 . Contact Elena Manzo at (209) 953 -7699 to arrange the following appointments ( a 4 & hour <br /> notice is required for inspection appointments ) . <br /> • Spill bucket test : Verify minimum 5 -gallon capacity and pass a one hour hydrostatic test <br /> ( or equivalent) . Proper drainage must also be verified . <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval . <br /> 4 . If concrete will be broken near the tank , a secondary containment test must be performed on <br /> the tank once concrete is poured in accordance with the manufacturer' s guidelines . <br /> 5 . Any individual performing the work of a service technician must meet the following <br /> qualifications : <br /> a . Possess or be employed by a person who possesses one of several permissible <br /> valid , current contractor' s licenses issued by the Contractors State License Board <br /> ( may be issued to the installation company) , or a Tank Tester' s license issued to the <br /> individual by the State Water Board , and <br />
