The following is an itemized list of hazardous materials violations that have not been <br /> addressed for WHOLESALE 209, DBA TIRE DIRECT as of January09, 2019. <br /> Open violations from December 07, 2018 inspection <br /> Violation #2 - Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> Violation#3 - Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br /> Plan shall be established and implemented for emergency response to a release or threatened release of a <br /> hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br /> materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br /> pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> enter the required information, and submit for review by the EHD. <br /> Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Three 55 gallon drums of synthetic oil, three above ground tanks of motor oil (one 150 gallons and two 250 gallons), <br /> two 55 gallon drums of waste antifreeze, and one 370 gallon above ground tank of waste oil were found on site that <br /> have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the facility's <br /> business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> Violation#6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Violation#7 -Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation#8 - Failed to submit a training program or program is not reasonable or adequate for facility. <br /> A training program was not submitted. Training programs for all employees and annual training, including refresher <br /> courses, shall include safety procedures for the event of a release or threatened release of a hazardous material, <br /> including, but not limited to, familiarity with the facility's emergency response plans and procedures. These training <br /> programs may take into consideration of each employee. Immediately prepare or update the facility's training <br /> program, log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
