Wrote complaint on white board next to Muniappa N.'s office. Notified Muniappa N. of the spill. 05/31/2018 Notification of Hazardous Waste Discharge. PE 2546 FLORIDO 30 min SC 203 05/31/2018 Generated Emergency Response Record Form and site map. PE 2546 FLORIDO 18 min SC 544 05/31/2018 On site and witnessed a pad mounted transformer with a Non-PCB sticker PE 2546 FLORIDO 78 min damaged with oil spillage on the soil surrounding the transformer. Met with Dave SC 544 Larsen of PG&E, who indicated the excavation of the area will begin on 6/4/2018 once the USA clears. Dave Larsen agreed to take a sample of the transformer oil to verify it is Non-PCB. PG&E to notify EHD when the excavation will take place in order to verify clean-up completion prior to backfill. PG&E to provide EHD with the sample analytical results prior to waste disposal, as well as final disposal records. Generated and emailed the Notice to Abate to Alex Steele of PG&E. 06/01/2018 Generated and printed photo log. PE 2546 FLORIDO 18 min SC 544 06/06/2018 Called and spoke to Alex Steele to get an update on the clean-up. Alex said the PE 2546 FLORIDO 6 min clean-up has not been performed yet, but will hopefully get done this week. Alex is SC 544 going to check in the Dave Larsen's schedule to find out when Dave will have time to do this clean-up. Alex indicated that he thins the oil sample analytical has been received, and he will provide it to me once he finds it. 06/06/2018 Received a call from Alex Steel who notified me Dave Larsen would likely conduct PE 2546 FLORIDO 12 min this clean-up tomorrow afternoon, given he is able to complete another clean-up in SC 544 the morning. Alex indicated he would let me know tomorrow morning. Received another call from Alex Steele notifying me the clean-up would not happen until Friday, since Dave Larsen has to take tomorrow off work. 06/08/2018 While I was witnessing the clean-up of another spill, Dave Larsen and Alex Steel PE 2546 FLORIDO 18 min of PG&E indicated that the clean up for this spill will be started today, but will likely SC 544 not be completed until tomorrow. I told Dave that our weekend on call person, Elena Manzo, will have to witness the clean-up. I told Dave I would let Elena know to give him a call to get a time as to when the clean-up will be completed. Dave agreed. I notified Elena of the clean-up to be completed tomorrow. I provided Elena with the complaint documentation and Dave Larsen's phone numbers. Elena indicated she would follow up with Dave to witness the clean-up. 06/08/2018 Follow up with Dave Larsen 209-401-9719, left a message asking for the ETA for PE 2546 MANZO 18 min clean up, provided my contact information. Received a call back from Dave, he SC 544 indicated that excavation will commence tomorrow. He will call me and let me know 2 hours in advance of being done. 06/08/2018 Called Dave Larson asked for the plan to clean up this spill, he indicated that the PE 2546 MANZO 12 min spill was soy bean oil and that he is planing to excavate tomorrow.
