The <br /> continuous monitoring system shall be maintained to detect the entry of the liquid or vapor phase of the hazardous <br /> substance stored in the primary containment into the secondary containment and to detect water intrusion into the <br /> secondary containment. The sensor was replaced and retested during the inspection. A"UST Retrofit Verification <br /> with Inspector Already Onsite" form has been completed and provided to the operator and contractor. This was <br /> corrected at the time of inspection. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91 submersible turbine sump (STP) sensor was positioned in a sensor holder located approximately 3-4 inches <br /> up the sump saddle and off the lowest point of the sump and not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> Immediately relocate this sensor to the lowest point of the 91 STP sump, submit proof or correction to the EHD. <br /> This is a Class II violation. <br /> 308 CCR 2636(f) Failed to maintain a continuous monitoring system that activates an audible and visual alarm. <br /> The 87 STP liquid sensor failed when tested. The continuous monitoring system shall be maintained to detect the <br /> entry of the liquid or vapor phase of the hazardous substance stored in the primary containment into the secondary <br /> containment and to detect water intrusion into the secondary containment. The technician onsite indicated that the <br /> sensor was stuck, he manipulated the sensor until it was moving freely and retested the sensor with passing results. <br /> This was corrected at the time of inspection. <br /> This is a Class II violation. <br /> 316 HSC 25290.1(c3), 25290.2(c3) Secondary containment not constructed to prevent water intrusion (after Jul 2003). <br /> The following VPH components were missing at the time of inspection: clamps for the installed shroud boots on all <br /> 87-octane spill containers, spill container shroud boots and clamps on the 91-octane and the diesel spill containers. <br /> Secondary containment shall be constructed to prevent any water intrusion into the system by precipitation, <br /> infiltration, or surface runoff. Immediately contact a properly licensed, trained, and certified contractor to address <br /> these missing secondary containment system components, submit proof of correction to the EHD. This is a repeat <br /> violation. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> This report was amended on 1-24-2019 and violations 113 and 123 were added. Complete and submit a copy of <br /> the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that <br /> have been or will be taken for each violation, and any supporting paperwork, 30 days from receipt of this <br /> amended inspection report. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
