The following is an itemized list of hazardous materials violations that have not been <br /> addressed for VALLEY FARMS TRANSPORT as of March25, 2019. <br /> Open violations from January 18, 2019 inspection <br /> Violation#5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> One 1,025 gallon tank of diesel exhaust fluid, one 10,000 gallon tank of diesel, one 110 gallon tank of waste oil, one <br /> 110 gallon tank of 15/40 oil, one 55 gallon drum of waste antifreeze, one 55 gallon drum of new antifreeze, two 55 <br /> gallon drums of 85/140 oil, one 55 gallon drum of 50 wt. oil, one 55 gallon drum of kerosene, three 100+ cubic foot <br /> cylinders of compressed oxygen (actual volume not available), and two 100+ cubic foot cylinders of acetylene <br /> (actual volume not available) were found on site that have not been reported. Any material that meets or exceeds <br /> the reportable quantity shall be reported in the facility's business plan. Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material <br /> inventory, and submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not completed or submitted as part of the business plan. A site map shall contain a north <br /> orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, <br /> emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency <br /> response equipment. If a site map element is not applicable for your facility then list it on the map and label as <br /> "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> upload the correct or updated information, and submit to the EHD for approval. <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to <br /> date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #11 - Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> Violation #12 -Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
