Geomatrix
March 21,2006
Mr.Harlin Knoll
San Joaquin County,Department of Environmental Health
Site Mitigation Unit
304 East Weber Avenue, 3d Floor
Stockton,CA 95202
Re: Request for Well Permit
Stormwater Infiltration Task
Sims Hugo Neu
1000 S. Aurora Street
Stockton,California
Dear Mr. Knoll:
Geomatrix Consultants Inc. (Geomatrix)has prepared this well permit application package for an
upcoming stormwater infiltration study. The objective of this work is to further define the extent
of volatile organic compounds (VOCs) in soil. To meet this objective, two locations within the
property boundaries have been selected for drilling. A single direct-push boring will be advanced
at each location to a total depth of approximately 20 feet below ground surface. The borings will
be continuously cored so as to facilitate lithologic profiling. Geomatrix does not anticipate
encountering groundwater. Each boring will be immediately backfilled with neat cement or a 4%
bentonite-grout mixture. Geomatrix has not developed a workplan for this task.
This permit package has been prepared in accordance with the requirements for well permitting
set forth by the San Joaquin County Environmental Health Department and consistent with
discussions between Harlin Knoll of the San Joaquin County Environmental Health Department
and Geomatrix personnel. The following items have been provided as attachments in this permit
application package for your use:
• One(1)completed Well Permit Application;
• Check in the amount of$89.00 addressed to San Joaquin County to cover the permit fee. The two
parcel property is owned by a single entity (this is pursuant to a 3/21106 phone conversation
between Harlin Knoll and Geomatrix regarding property ownership status);
• Completed Master File Record information for the project;
• Licensed Contractors Declaration and certificate of workers Compensation Insurance for from the
drilling subcontractor(RSI);and
• Check in the amount of$279.00 addressed to San Joaquin County to cover the site mitigation fee.
10670 White Rock Road,Suite 100 Tel 916.636.3200 www.geomatrix.com
Rancho Cordova,California 95670-6032 Fax 916.636.3208
