I have included a copy of the <br /> "Cost Pre-Approval Request" form; please use this form in the future for requesting pre-approval <br /> of corrective actions costs. <br /> With the following provisions, the total cost pre-approved as eligible for reimbursement for <br /> completing the March 25, 1999, JJW Geosciences, Inc. workplan approved by the San Joaquin <br /> County EHD (County) in their April 1, 1999 letter, is $2,965. This pre-approval is for drilling <br /> and sampling of one pilot hole to 70 feet.
