PHSIEHD also received a permit application <br /> and$89 check from Aqua Geosciences. <br /> Please note that the last correspondence which PHSIEHD received from Pieter Crosby dated May 12, 1997, <br /> indicated that he would be submitting a new work plan. It was surprising then to receive a permit application <br /> without a cover letter explaining that the permit application was for work already approved by PHS/EHD in <br /> correspondence dated March 27, 1996. PHS/EHD subsequently contacted Pieter Crosby,who stated that the <br /> approved work plan would be implemented. PHS/EHD then contacted the C-57 license holder, Gregg Drilling,to <br /> obtain authorization for the Aqua Geosciences representative to sign the permit application. Chris Palmer had no <br /> knowledge about the application submitted and requested that a copy of the application be faxed for his review. <br /> PHSIEHD has only received faxed copies of analytical results for stockpile soil samples collected on November 8, <br /> 1996. As background,PHSIEHD received on November 7, 1996 notification by Pieter Crosby that Lush <br /> Environmental and yourself would be onsite at 9:00 AM on November 8, 1996 to collect the soil samples. <br /> PHSIEHD staff waited on site until approximately 10:00 AM for the Lush representative to arrive and when he did <br /> so,he was unprepared to sample. PHSIEHD waited as long as possible, almost 11:00 AM, for the sampling <br /> equipment to arrive,but was unable to witness the soil sampling because of scheduling conflicts. A stockpile soil <br /> sampling inspection generally is accomplished within 30 minutes. PHSIEHD staff explained how the samples <br /> should be collected and was willing to accept the results,since the soil was to be placed into the former tank pits to <br /> accommodate the proposed drilling activities. PHSIEHD will accept the stockpile sample results if they are <br /> submitted in original,but will retain the right to further evaluate the backfill based on field observations. <br /> PHSIEHD will issue the permit once authorization from the C-57 license holder is obtained and clarification <br /> regarding the proposed work is submitted.
