PART VI - DESCRIPTION OF EVENTS & PACKAGE FAILURE <br /> Describe the sequence of events that led to the incident and the actions taken at the time it was discovered. Describe the package failure, <br /> including the size and location of holes,cracks,etc.Photographs and diagrams should be submitted if needed for clarification.Estimate <br /> the duration of the release,if possible. Describe what was done to mitigate the effects of the release.Continue on additional sheets if <br /> necessary. <br /> On 8/21/15 at approximately 00:02 PST,the UTLX 200074 was found with wetness on the side of the car and a small amount dripping from the car. <br /> BNSF Hazmat responders and contractors responded to the location and found the Liquid Line Flange bolts were less than tool tight resulting in <br /> spillage from the appurtenance. The fitting was tightened appropriately and spillage was cleaned from the car and impacted soils removed and <br /> stored for future disposal pending profiling and acceptance. There were no injuries,exposures or evacuations reported. <br /> PART VII - RECOMMENDATIONS/ACTIONS TAKEN TO PREVENT RECURRENCE <br /> Where you are able to do so,suggest or describe changes(such as additional training,use of better packaging,or improved operating <br /> procedures)to help prevent recurrence.Provide recommendations for improvement to hazardous materials transportation beyond the <br /> control of your individual company.Continue on additional sheets if necessary. <br /> Shipper contact made to Tessenderlo Kerley(Burley,ID)to ensure that loading personnel are properly inspecting and securing rail car shipments <br /> prior to being offered for transportation.
