AUGI 2015 <br /> ❑ SUPPLEMENrAL REPORT tt��II IRO' pp <br /> ❑ OTHER: _ 9VVIRO' IVl��TA <br /> r <br /> I called EHS Naidu contact number leaving my information and to call if he would care for my assistance at 9:06 <br /> AM. His voice mail left a number to contact the Environmental Health office. I called the number given and spoke with <br /> Kim Blackwell. Ms. Blackwell stated the Specialist to speak with was Elaina Manzo, Specialist Manzo was in the field <br /> and aware of the spill. I was informed my name and number would be passed on and I would be contacted if needed. I was <br /> given Specialist Manzo's number. I called the number and received voicemail. The voicemail gave a number to call if <br /> immediate service is needed. I called the number(209-953-6275)and spoke with Lowell Allen. Mr. Allen stated he <br /> would contact EGS Naidu and give him my information. Mr.Allen then gave me the general location of the railcar <br /> leakage. <br /> At 10:02 AM I drove north to the end of Diamond Street. Diamond Street ends at The BNSF Mormon Rail Yard. I <br /> contacted Railman Andrea(no last name). He asked for my identification and reason for being there, he was unaware of <br /> the spill. He contacted his supervisor Train Master Don Obraovid. Railman Andrea stated I need to go speak to The Train <br /> Master. I was instructed to go to the train tower that is located off of Hazelton Road, I arrived at the train tower at 10:15 <br /> AM. I spoke with Train Master's Joe Brice and Don Obraovid. I informed them I was with the Agricultural <br /> Commissioner's Office and needed information on the spill. I requested to speak to anyone present during the spill. I was <br /> informed it occurred around 12:00 AM all personnel present at the time of the spill were gone. Mr. Obraovid stated to his <br /> knowledge no one felt ill or reported having symptoms as a result of the spill. <br /> Mr. Obaovid stated last night around 12:00 AM(August 21,2015)a rail man,he did not have his name, saw and reported a <br /> tank car leaking. As a result and following BNSF emergency work plan the Train Master on site pulled all personnel <br /> indoors until the substance spill was identified. He stated once identified it is determined what a safe radius prior to <br /> anyone being allowed to approach the railcar. The substance was determined to be Metam Sodium. Mr. Obraovid printed <br /> a copy of the Hazmat list for Metam Sodium, giving me a copy. He then showed me on a monitor the location of railcar <br /> UTLX 200074 and the location where it was to be moved. <br /> I then received a call from EHS Naidu. I informed him of the information I had obtained and the location of the <br /> railcar. He stated his office had not responded yet, and if we could meet. I informed him I was on my way to the rail car <br /> location off of Diamond Street.Mr.Naidu inquired as to the nature of Metam Sodium,was it a gas or a liquid. I informed <br /> him it was a liquid used in agriculture for soil fumigation against soil pests. <br /> Prior to the arrival of EHS Naidu I contacted a BNSF Railman to inquire as to the location of track 139 and track <br /> 117. Track 117 is the location Mr. Obraovid stated the leaking railcar would be moved too.At approximately 10:50 AM <br /> EHS Naidu and Hazardous Materials Specialist 1 (HMS)Haza Saeed arrived. <br /> EHS Naidu,HMS Saeed and I walked west in seek of the tank car. The tank car was being moved into place on <br /> track 117. The tank car was black and identified with yellow letters UTLX 200074. Located on the center top of the rail <br /> car was valves with caution tape(see photo). It appears to be the location of the leakage. A label for Sectagon 42 was <br /> attached to the tankcar. Sectagon 42 has the signal word"DANGER"with EPA registration number 61842-6. It is a <br /> product of Nova Source. The spill appeared to occur on only one side of the car(north side at its present location). <br /> While located on the tracks we were approached by Railman Mike Garcia. We identified ourselves and asked to be <br /> taken to the location of the spill.Mr. Garcia stated he was unaware of a spill and would have to obtain permission before <br /> he took us to the requested location. I informed Mr. Garcia I spoke with Train Masters Obraovid and Brice. He left for <br /> approximately 30 minutes. He returned stating he could take us to the location of the spill. Before allowing us to enter the <br /> train and track area he went over safety rules. We were given access to the location of the spill. There was no significant <br /> signs a spill had occurred. Mr. Garcia stated he was told very little spilled on the ground,he stated a pan was placed under <br /> the car to reduce soil contamination but very little got on the pan. <br /> The Metam Sodium spill occurred from a rail car during transit.
