ACCESS AGREEMENT Monitoring Well Destruction Taco Bell, 6425 West Banner Street, Lodi, California (APN: 055-320-20) As the property owner, I do hereby grant permission to Advanced GeoEnvironmental, Inc. (AGE) to destroy ground water extraction wells EW-4 and EW-5, located at 6425 West Banner Street, Lodi, California (APN: 055-320-20), on behalf of Flag City Shell (Gil Moore Oil Company). The scope of work at 6425 West Banner Street will include the destruction of wells EW-4 and EW-5 by pressure grouting techniques. The well box and cap will be removed to expose each well casing, and then a thick grout mixture will be pumped into the well casing under pressure. Following application of the grout, the well will be over-drilled to approximately 3 feet belowsurface grade(bsg)using 10-inch diameter hollow stem augers. The top 2.5 feet of the well casing will then be cut and removed and grout will be used to fill the boring to approximately 0.5 feet above the top of the well casing. The remaining borehole will be backfilled to surface grade with neat cement and covered with concrete or asphalt, depending on the surrounding surface material.The entire process will take an estimated three to four hours for complete destruction of both wells. Prior to well destruction activities, AGE will submit this Access Agreement and a Well Destruction Permit Application to the San Joaquin County Environmental Health Department (EHD) for procurement of a Well Destruction Permit. All expenses for the well destruction activities at the 6425 West Banner Street site shall be paid for by Flag City Shell (Gil Moore Oil Company)and will be conducted under permit in a timely manner upon approval by the San Joaquin County Environmental Health Dept.
