ACCESS AGREEMENT <br /> Monitoring Well Destruction <br /> Flag City Chevron, 6421 Capitol Avenue, Lodi, California <br /> (APN: 055-320-24) <br /> As the property owner, I do hereby grant permission to Advanced GeoEnvironmental, Inc. <br /> (AGE) to destroy ground water extraction well EW-10A, located at 6421 Capitol Avenue, <br /> Lodi, California (APN: 055-320-24), on behalf of Flag City Shell (Gil Moore Oil Company). <br /> The scope of work at 6421 Capitol Avenue will include the destruction of well EW-10A by <br /> pressure grouting techniques. The well box and cap will be removed to expose the well <br /> casing, and then a thick grout mixture will be pumped into the well casing under pressure. <br /> Following application of the grout, the well will be over-drilled to approximately 3 feet below <br /> surface grade (bsg) using 10-inch diameter hollow stem augers.The top 2.5 feet of the well <br /> casing will then be cut and removed and grout will be used to fill the boring to <br /> approximately 0.5 feet above the top of the well casing. The remaining borehole will be <br /> backfilled to surface grade with neat cement and covered with concrete or asphalt, <br /> depending on the surrounding surface material. The entire process will take an estimated <br /> two to three hours for complete destruction of the well. <br /> Prior to well destruction activities, AGE will submit this Access Agreement and a Well <br /> Destruction Permit Application to the San Joaquin County Environmental Health <br /> Department (EHD) for procurement of a Well Destruction Permit. <br /> All expenses for the well destruction activities at the 6421 Capitol Avenue site shall be paid <br /> for by Flag City Shell (Gil Moore Oil Company) and will be conducted under permit in a <br /> timely manner upon approval by the San Joaquin County Environmental Health Dept. <br /> All affected tenants and property owners of the site will be given 1-week notice prior to the <br /> initiation of field activities, which will be conducted in a manner to minimize disruption of <br /> normal site activities. <br /> 40d Family Limited PTP, Property Owner 1411 <br /> Woodbridge, California 95258 <br /> APN: 055-320-24 <br /> Advanced Gcol nvironmental,Inc. <br />
