ACCESS AGREEMENT <br /> Monitoring Well Destruction <br /> Microtel Inn & Suites, 6428 West Banner Street, Lodi, California <br /> (APN: 055-320-45) <br /> As the property owner, I (Rajiv M. & Rajni Jain,) do hereby grant permission to Advanced <br /> GeoEnvironmental, Inc. (AGE)to destroy ground water monitoring wells MW-4A, MW-4B, <br /> MW-4C, MW-7A, MW-7B, EW-6A, EW-7A and EW-713, located at 6428 West Banner <br /> Street, Lodi, California (APN: 055-320-45), on behalf of Flag City Shell (Gil Moore Oil <br /> Company). <br /> The scope of work at will include the destruction of wells MW-4A, MW-4B, MW-4C, <br /> MW-7A, MW-7B, EW-6A, EW-7A and EW-7B by either pressure grouting or drill-out <br /> techniques. Each traffic rated well box and well cap will be removed to expose the well <br /> casing. <br /> For pressure grouting, a thick grout mixture will be pumped into the well casing under <br /> pressure for 20 minutes. The remaining space will be backfilled to surface grade with neat <br /> cement and covered with concrete or asphalt, depending on the surrounding surface <br /> material. The entire process will take an estimated two to three hours for completion. <br /> For drill-outs, each well will be over-drilled to total depth using 8-inch or 10-inch diameter <br /> hollow stem augers. The entire contents of the well will be removed. The remaining <br /> borehole space will be backfilled to surface grade with neat cement and covered with <br /> concrete or asphalt, depending on the surrounding surface material. The entire process <br /> will take an estimated three to four hours for completion. <br /> Prior to well destruction activities, AGE will submit a Master File Record Information form <br /> and a Well & Boring Permit Application to the San Joaquin County Environmental Health <br /> Department (EHD) for procurement of a Boring Permit. <br /> All expenses for the well destruction activities at the 6428 West Banner Street site shall be <br /> paid for by Flag City Shell (Gil Moore Oil Company) and will be conducted under permit in <br /> a timely manner upon approval by the San Joaquin County Environmental Health Dept. <br /> All affected tenants and property owners of the site will be given 1-week notice prior to the <br /> initiation of field activities, which will be conducted in a manner to minimize disruption of <br /> normal site activities. <br /> R j v t n <br /> M. i Jain, Property Owner Date <br /> 428 West Banner Street <br /> Lodi, California 95242 <br /> APN: 055-320-45 <br /> Advaeced GeoEnvironmenlal.Inc. <br />
