ACCESS AGREEMENT Monitoring Well Destruction Flag City RV Resort, 6120 West Banner Street, Lodi, California (APN: 055-320-57) As the property owner of Flag City RV Resort, I do hereby grant permission to Advanced GeoEnvironmental, Inc. (AGE) to destroy ground water extraction wells MW-9B, MW-9C and MW-9D, located at 6120 West Banner Street, Lodi, California (APN: 055-320-57), on behalf of Flag City Shell (Gil Moore Oil Company). The scope of work at 6120 West Banner Streetwill include the destruction of wells MW-9B, MW-9C and MW-9D by pressure grouting techniques. The well box and cap will be removed to expose each well casing, and then a thick grout mixture will be pumped into the well casing under pressure. Following application of the grout, the well will be over-drilled to approximately 3 feet below surface grade (bsg) using 10-inch diameter hollow stem augers. The top 2.5 feet of the well casing will then be cut and removed and grout will be used to fill the boring to approximately 0.5 feet above the top of the well casing. The remaining borehole will be backfilled to surface grade with neat cement and covered with concrete or asphalt, depending on the surrounding surface material. The entire process will take an estimated three to four hours for complete destruction of all three wells. Prior to well destruction activities, AGE will submit this Access Agreement and a Well Destruction Permit Application to the San Joaquin County Environmental Health Department (EHD) for procurement of a Well Destruction Permit. All expenses for the well destruction activities at the 6120 West Banner Street site shall be paid for by Flag City Shell (Gil Moore Oil Company) and will be conducted under permit in a timely manner upon approval by the San Joaquin County Environmental Health Dept. All affected tenants and property owners of the site will be given 1-week notice prior to the initiation of field activities, which will be conducted in a manner to minimize disruption of normal site activities.
