Aug 15 12 10:25a Woodbridge Fir*trict 19694568 p.1 <br /> ACCESS AGREEMENT <br /> Monitoring Well Destruction <br /> Delta Fire Department, 6351 West Capitol Avenue, Lodi, California <br /> (APN: 055-320-46) <br /> As the property owner, the Woodbridge Rural Fire District does hereby grant permission <br /> to Advanced GeoEnvironmental, Inc. (AGE) to destroy ground water monitoring wells <br /> MW-8A and MW-813, located at 6351 West Capitol Avenue, Lodi, California (APN: 055- <br /> 320-46), on behalf of Flag City Shell (Gil Moore Oil Company). <br /> The scope of work at will include the destruction of wells MW-8A and MW-81B by either <br /> pressure grouting or drill-out techniques. Each traffic rated well box and well cap will be <br /> removed to expose the well casing. <br /> For pressure grouting well MW-8A, a thick grout mixture will be pumped into the well casing <br /> under pressure for 20 minutes.The remaining space will be backfilled to surface gradewith <br /> neat cement and covered with concrete or asphalt, depending on the surrounding surface <br /> material. The entire process will take an estimated two to three hours for completion. <br /> For drilling-out, well MW-88 will be over-drilled to total depth of the well using 8-inch <br /> diameter hollow stem augers. The entire contents of the well will be removed. The <br /> remaining borehole space will be backfilled to surface grade with neatcement and covered <br /> with concrete or asphalt, depending on the surrounding surface material. The entire <br /> process will take an estimated three to four hours for completion. <br /> Prior to well destruction activities, AGE will submit this Access Agreement and a Well <br /> Destruction Permit Application to the San Joaquin County Environmental Health <br /> Department (EHD) for procurement of a Well Destruction Permit. <br /> All expenses for the well destruction activities at the 6440 West Banner Street site shall be <br /> paid for by Flag City Shell (Gil Moore Oil Company) and will be conducted under permit in <br /> a timely manner upon approval by the San Joaquin County Environmental Health Dept. <br /> All affected tenants and property owners of the site will be given 1-week notice prior to the <br /> initiation of field activities, which will be conducted in a manner to minimize disruption of <br /> n mal site activities. <br /> Woodbridge Rural Fire Dept., Property Owner Date <br /> 400 East August Street <br /> Lodi, California 95258 <br /> APN: 055-320-46 <br /> Advanced CcoEnvironmcnial,Inc. <br />
