(AGE)has prepared this reportforthe destruction of thirty-one(31) <br /> groundwater monitoring wells and eleven (11) groundwater extraction wells at 6437 West <br /> Banner Street, Lodi, California (site). The site location is depicted on Figure 1; site <br /> structures and former well locations are depicted on Figure 2. <br /> INTRODUCTION/BACKGROUND <br /> Groundwater monitoring and extraction wells had previously been installed at the site as <br /> part of a leaking underground storage tank (UST) subsurface investigation and <br /> remediation. A total of thirty-one (31) two-inch diameter groundwater monitoring wells <br /> (MW-1, MW-2, MW-3A/B/C, MW-4A/B/C,MW-5A/B, MW-6B/C, MW-7A/B, MW-8A/B, MW_ <br /> 96/Cou Mbeen installed <br /> MWd116, MW-12A, MW-13A/B, MW-19A/B and MW-20A/B) had <br /> previously been installed to depths ranging from 25 to 91.5 feet below surface grade (bsg). <br /> Additionally, eleven 01)four-inch and six-inch groundwater extraction wells(EW-1 through <br /> EW-5, EW-6A, EW-7A, EW-713, EW-813, EW-96 and EW-10A) were installed at depths <br /> between 25 and 50 feet bsg (Table 1). <br /> Well destructions were conducted in September and October 2012 as outlined in the AGE- <br /> prepared Monitoring We//Destruction Work Plan, dated 24 July 2012. The work plan was <br /> prepared in accordance with California Regional Water Quality Control Board - Central <br /> Valley Region (CRWQCB-CVR) directive letter, dated 31 January 2012.
