(Closure Solutions) is submitting this <br /> letter to notify you that well MW-11B has been repaired at the New West—Flag City facility, <br /> located at 6437 West Banner Street, in Lodi,California(the Site, Figure 1). Below is a summary <br /> Of the events that took place to repair the damaged well. <br /> On April 21, 2008, you informed us that Mr. Mike Infurna reported that the well had been <br /> damaged, and that the well was not secure. Due to the projected upcoming rains, you requested <br /> the well be repaired as soon as possible. <br /> On April 21, 2008, Confluence Environmental, Inc. (Confluence) evaluated the well condition. <br /> To temporarily secure the well, Confluence placed a coupling over the exposed casing and <br /> capped it. Confluence then filled the well box with asphalt cold patch to the surface grade to <br /> temporarily secure the wellhead against groundwater runoff and protect against tampering. <br /> On April 21, 2008, Closure Solutions notified you of the temporary measures undertaken to <br /> secure the well. During this conversation Closure Solutions indicated that the well may be <br /> repaired as soon as Thursday, April 24, 2008, however after further consideration, Closure <br /> Solutions concluded that a traffic control permit would be necessary to complete the well repair <br /> and well box replacement. <br /> On May 9, 2008, after securing the appropriate traffic control peanuts, Confluence repaired the <br /> well and replaced the well box. The damaged PVC casing was cut down to level the casing and <br /> a 2" PVC coupler and 2" blank PVC casing was attached to the remaining well casing.
