(New West), Closure Solutions, Inc. (Closure Solutions) is <br /> submitting this letter report to document the destruction and reinstallation of monitoring well <br /> MW-IOD at the Flag City New West facility located at 6437 West Banner Street, in Lodi, <br /> California. <br /> Monitoring well MW-101) was initially installed on September 18, 2007 at a location roughly <br /> 100 feet south of the intersection of Star Street and Capitol Avenue (Figure 1). The well was <br /> installed by Gregg Drilling & Testing of Martinez, California. During the course of well <br /> development, on October 3, 2007. Blaine Tech Services of Sacramento, California reportedly <br /> encountered a blockage in the well, and the surge block became permanently lodged in the <br /> casing. <br /> Because the surge block prevented access to groundwater, Closure Solutions elected to properly <br /> destroy the well, and reinstall MW-IOD. Based on a discussion with San Joaquin County <br /> Environmental Health, because the well was merely being reinstalled in the same borehole, an <br /> additional drilling permit did not need to be obtained. An Encroachment Permit Extension was <br /> obtained from the County prior to initiating work. <br /> On November 30, 2007, Closure Solutions oversaw the reinstallation of monitoring well MW- <br /> 1 OD. The original well casing was properly abandoned using a 10" diameter hollow stem auger, <br /> and the new well was reinstalled in the borehole.
