This facility does not have a valid EPA ID number to manage hazardous waste. The manager <br /> claims to get a temporary EPA ID number every time they need to dispose hazardous waste. A <br /> hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, <br /> hazardous_waste_without an.EPA ID number., Immediately obtain an EPA ID number to manage <br /> hazardous waste and submit evidence to the EHD within 30 days of receiving this report. A form has <br /> been provided. <br /> 605 & 701. A blue metal 500 gallon tank storing used oil had a hazardous waste label that had faded <br /> out. All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since these containers have been on site longer than 180 days, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the . <br /> manifest to the EHD within 30 days of receiving this report. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br /> Inspector.
