Disposal record such as manifest or consolidated manifest for the disposal of hazardous waste <br /> and filters must be kept on site for at least three years. Locate the disposal records for past three years <br /> and submit to this office to determine the generator status. <br /> Notes <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, to the EHD within 30 days of <br /> receiving this report. <br /> Facility lacked contingency plan.
