0 0 <br /> The following is an itemized list of hazardous waste violations that have not been <br /> addressed for ARAMARK UNIFORM & CAREER APPAREL LLGSTOCKTON as of <br /> February 13, 2018. <br /> Open violations from May 13, 2015 inspection <br /> Violation#102 -Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br /> years. <br /> Four 55-gallon poly containers, three 55-gallon red metal containers, six 5-gallon plastic containers, and two <br /> 5-gallon open containers (with some black oily liquid)were located adjacent to the chemical storage area, were <br /> unlabeled, and at the time of inspection lacked a hazardous waste determination. Any person who generates a <br /> waste shall determine if the waste is a hazardous waste. Immediately make a hazardous waste determination for <br /> the noted containers and manage them according the Title 22 hazardous waste regulations. Submit a statement <br /> and supporting documentation explaining how this waste was managed. <br /> Violation#104-No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. <br /> Violation#117-Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Violation#301 -Facility not maintained to minimize the release of a hazardous waste. <br /> Oily staining and pooled oil were noted inside the secondary containment and on the floor around one oil filled <br /> piece of equipment located inside the chemical storage area. Furthermore, white crystal deposits and floor <br /> corrosion were noted around a large tank also located in the chemical storage area. Facilities shall be maintained <br /> and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface <br /> water which could threaten human health or the environment. Immediately fix all leaks and staining and manage <br /> the recovered waste according to Title 22 hazardous waste regulations. Submit a statement and supporting <br /> documentation explaining how this waste was managed. <br /> Violation#303 - Failed to maintain all communication or alarm systems,spill control,or decontamination <br /> equipment. <br /> An eye wash station located in the chemical storage area was not maintained. All facility communications or alarm <br /> systems,fire protection equipment, spill control equipment, and decontamination equipment, where required, shall <br /> be tested and maintained as necessary to assure its proper operation in time of emergency. Immediately begin this <br /> practice. <br /> Violation#403 -Failed to keep hazardous waste containers closed except when adding or removing <br /> hazardous waste. <br /> Two 5-gallon open containers (one red and one green)with approximately 1-inch of oily liquid and oily rags were <br /> stored alongside several unlabeled hazardous waste containers noted in the previous violation. All hazardous <br /> waste containers shall be closed at all times except when adding or removing waste. Immediately close these <br /> containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br /> Page 1 of 2 <br />
