0 0 The following is an itemized list of hazardous waste violations that have not been addressed for ARAMARK UNIFORM & CAREER APPAREL LLGSTOCKTON as of February 13, 2018. Open violations from May 13, 2015 inspection Violation#102 -Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 years. Four 55-gallon poly containers, three 55-gallon red metal containers, six 5-gallon plastic containers, and two 5-gallon open containers (with some black oily liquid)were located adjacent to the chemical storage area, were unlabeled, and at the time of inspection lacked a hazardous waste determination. Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a hazardous waste determination for the noted containers and manage them according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. Violation#104-No modified contingency plan. An emergency coordinator and modified contingency plan information is lacking. There must be at least one emergency coordinator on site or on call to coordinate emergency response measures, and the following information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. A form is provided that can be used for this purpose. Violation#117-Failed to report program data electronically. A submission to the California Electronic Reporting System (CERS)for the hazardous waste program has not been made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required fields. Violation#301 -Facility not maintained to minimize the release of a hazardous waste. Oily staining and pooled oil were noted inside the secondary containment and on the floor around one oil filled piece of equipment located inside the chemical storage area. Furthermore, white crystal deposits and floor corrosion were noted around a large tank also located in the chemical storage area. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately fix all leaks and staining and manage the recovered waste according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. Violation#303 - Failed to maintain all communication or alarm systems,spill control,or decontamination equipment. An eye wash station located in the chemical storage area was not maintained. All facility communications or alarm systems,fire protection equipment, spill control equipment, and decontamination equipment, where required, shall be tested and maintained as necessary to assure its proper operation in time of emergency. Immediately begin this practice. Violation#403 -Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. Two 5-gallon open containers (one red and one green)with approximately 1-inch of oily liquid and oily rags were stored alongside several unlabeled hazardous waste containers noted in the previous violation. All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. Page 1 of 2
