SAN _JOAQUIN Environmental Health Department <br /> -COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: JFacility Address: Date: <br /> D C AUTO REPAIR 1405 E WEBER AVE, STOCKTON February 05, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures have not been submitted. A business plan shall include the following <br /> emergency response procedures for a release or threatened release of hazardous materials, including, but not <br /> limited to, the following: <br /> -Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually. The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures,the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section. Immediately make arrangements to have the business <br /> plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the completed inspection report. <br /> Business has been in operation at this location since 2018. <br /> Owner operated; no employees. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
