Benjamin Holt Dr. <br /> Stockton, CA 95207 <br /> On March 23 , 194, Shell Oil Company was notified that the above <br /> referenced site was placed into the Local Oversight Program (LOP) <br /> for evaluation of assessment and clean up activities at this <br /> contaminated underground tank (UGT) site. Further investigation <br /> is required to determine if groundwater beneath your site has <br /> been impacted. No workplan has been submitted addressing the <br /> proposed next step of investigation and San Joaquin County Public <br /> Health Services, Environmental Health Division (PHS/EHD) hereby <br /> directs you to submit to our office a "Preliminary Investigation <br /> and Evaluation Report" (PIER) by October 30, 1992. <br /> Please follow requirements for workplan and report submittals <br /> detailed in "Tri Regional Board Staff Recommendations for Initial <br /> Evaluation and Investigation of Underground Tanks, Appendix A- <br /> Reporting" and the LUFT manual.
