(Stantec) is <br /> submitting this revised well destruction work plan for the Chevron Facility No. 94275 and ARCO <br /> Service Station No. 2133 located at the above referenced address (the "Site; Figure 1). The <br /> well destruction work plan was requested from the San Joaquin County Environmental Health <br /> Department (SJCEHD) in a letter dated November 6, 2012 (Attachment A) as part of site <br /> closure activities. The Site has been part of an environmental investigation case under the <br /> direction of the SJCEHD. This revised work plan presents a detailed scope of work proposed for <br /> monitoring and remediation well destruction. Well construction details and the proposed well <br /> destruction method for each well are included in Table 1. <br /> The Well Destruction Work Plan dated April 10, 2012, was submitted as an attachment in the <br /> No Further Action Request Report dated April 10, 2012. The April 10 work plan incorrectly <br /> indicated that locations AS-8 through AS-11 were wells that needed be destroyed. Locations <br /> AS-8 through AS-11 were drilled as soil borings and as such properly abandoned at the time of <br /> drilling. This work plan has been revised to remove locations AS-8 through AS-11 from the well <br /> destruction scope of work. No other revisions have been made to the April 10 work plan. <br /> Site Health and Safety Plan <br /> As required by the Occupational Health and Safety Administration (OSHA) "Hazardous Waste <br /> Operations and Emergency Response" guidelines (29 CFR 1910.120) and by California <br /> Occupational Health and Safety Administration (Cal-OSHA) "Hazardous Waste Operations and <br /> Emergency Response" guidelines (CCR Title 8, Section 5192), the contractor will prepare a site <br /> specific Health and Safety Plan (HASP). At a minimum, the HASP will define the proposed <br /> subsurface investigation activities, describe physical and chemical hazards that may be <br /> associated with the work, provide a map to the nearest emergency medical facility, and include <br /> material safety data sheets for any hazardous chemicals that will be used or produced during <br /> the work. A copy of the HASP will be available onsite at all times during field work. The field <br /> staff and contractors performing field activities will review the HASP prior to beginning field <br /> operations, as described in this work plan, at the site. <br /> Revised Well Destruction Work Plan 2012.docx 1 <br />
