Box 388 • Stockton, CA 9 x'83-0388 <br /> 209/468-3420 J <br /> �. <br /> BRETT HUNTER <br /> ENVIRONMENTAL ENGINEER MAULED NOV 3 01995 <br /> CHEVRON USA <br /> P O BOX 5004 <br /> SAN RAMON CA 94583-0804 <br /> RE: Lincoln Village Chevron Station 49-4275 SITE CODE: 1952 <br /> 2905 West Benjamin Holt <br /> Stockton CA 95207 <br /> San Joaquin County Public Health Service, Environmental Health Division (PHS/EHD) has reviewed the <br /> following reports which were submitted with Chevron correspondence dated September 18, 1995: <br /> "Fourth Quarter 1994 Monitoring Report" dated February 8, 1995 and prepared by Blaine Tech; <br /> "First Quarter 1995 Monitoring Report" dated March 28, 1995 and prepared by Blaine Tech; <br /> "Second Quarter 1995 Monitoring Report" dated July 13, 1995 and prepared by Blaine Tech; and <br /> "Work Plan for Soil and Groundwater Remediation Services" dated August 31, 1995 which was <br /> prepared by Geraghty and Miller. <br /> PHS/EHD has the following comments for your consideration. <br /> The results of groundwater sampling has continued to evidence significant groundwater contamination <br /> which requires remediation. Please note that quarterly reports must be submitted in a timely manner. <br /> Quarterly reports shall be submitted within 90 days of the date of sampling so that PHS/EHO can evaluate <br /> recent site conditions. <br /> Background <br /> PHS/EHD notified you in correspondence dated April 7, 1994 that the problem assessment report (PAR) <br /> which was submitted, indicated that feasibility studies would be necessary to evaluate the effectiveness of <br /> bio-spar-ing, vapor extraction; and air sparging with vapor extraction. PHS/EHD further stated that field <br /> work including feasibility testing should be completed within 45 days of PHS/EHD issuance of a drilling <br /> permit and that an addendum to the PAR should be submitted by July 15, 1994 which indicates the most <br /> efficient remedial alternative for the site. <br /> PHS/EHD notified you in correspondence dated November 7, 1994 that the corrective action plan, <br /> justifying the selected remedial alternative with feasibility studies and cost effective analysis, should be <br /> submitted by December 30, 1994. In Chevron correspondence dated December 1, 1994, you indicated <br /> that a corrective action plan would be submitted by December 30, 1994. <br /> PHS/EHD received Chevron correspondence dated January 30, 1995 requesting an extension for the <br /> submittal of a corrective action plan until May 15, 1995. During a telephone conversation with you on <br /> February 3, 1995, PHS/EHD requested that the selected remedial alternative be submitted before initiating <br /> the bidding process. PHS/EHD received a phone message on May 30, 1995 indicating that a corrective <br /> action plan would be submitted by June 14, 1995. PHS/EHD was subsequently requested to contact Kent <br /> O'Brien with Geraghty and Miller for status updates for the corrective action plan. Mr. O'Brien was <br /> contacted and PHS/EHD was notified that the engineering design would be completed by June 29, 1995, <br /> after which time a meeting would be scheduled to discuss the draft completed. PHS/EHD received the <br /> above referenced reports on September 20, 1995. <br /> A Division or San Joaquin County Health Care Services <br />
