SANJOAQUINEnvironmental Health Department <br /> COUNTY <br /> Complaint Record Daily Time and Activity Summary from 6/21/2013 to <br /> 6/21/2020 for COEDRQ2NL located at <br /> Activity Date Recorded by <br /> 12/18/2017 At 8:00 am I called Loren Shelton with CalTrans and asked him if he is on site for PE 2546 SAEED <br /> 2132 min the cleanup, and told me Juan Armendariz will be there. I called Juan and told me SC 544 <br /> he is on his way. I asked Juan if they notified PG&E of the cleanup and he was <br /> not sure. At 08:10am I left the office responding to the incident. At 8:27am I <br /> arrived onsite and met with Juan Rubio and Ty Reguera with Patriot <br /> Environmental. Patriot was setting up the equipment to start the cleanup. I <br /> inspected the area and took photos before they started the cleanup. At 8:38am <br /> Juan with CalTrans arrived onsite. At 08:45am I called Rob Cosmero with PG&E <br /> to notify him of the cleanup and letting him know that the cleanup will be close to <br /> the pole. Rob told me he is the supervisor in Jackson area and I need to call Dave <br /> Snowdem at(209)471-0057. 1 called Dave, and Dave told me the incident <br /> happened last weekend and no one notified them that the cleanup will be <br /> conducted today. He told me that he will try to find someone to respond and help <br /> holding the pole. I discussed the waste disposal with Ty and told me the waste <br /> will be placed in bins and will be taking to CalTrans yard until the waste <br /> profile/analysis is done. Dave Snowdem called me back and told me his crew will <br /> be on site in about one hour. I told him I told Patriot not dig deep around the pole <br /> and support until you arrive, and if we think we do not need PG&E to come , we <br /> will let you know. Dave told me the crew already on their way. At 9:30am PG&E <br /> crew arrived on site, and Juan with Patriot told them to stay around, but we may <br /> not need them. Juan with CalTrans asked me if I know the property owner <br /> information for the field next to their property. I told him now, but I can Elena and <br /> ask. I did not have access to Parcel Viewer, I called Elena and left a voice mail to <br /> call me back. <br /> I kept inspecting the cleanup, and told Juan with CalTrans that more cleanup <br /> required, and additional excavation was done. Final cleanup was completed It <br /> 11:10 am. I inspected, took photos of the cleanup areas, waste bins, and waste <br /> label. Three 20 yards bins of waste were generated. I completed the narratives, <br /> and left the site at 11:35am. <br /> at 2:00pm I completed the photo log for the photos I took. I looked up the adjacent <br /> property to CalTrans property on Parcel viewer and emailed the information to <br /> Juan with CalTrans. <br /> 01/02/2018 Emailed the responsible party, and copied Juan Rubio and Ty Reguera with PE 2546 SAEED <br /> 12 min Patriot Environmental to send me the disposal records. Ty replied " SC 002 <br /> Haza the bins are still onsite, this week we will probably start moving the bins out. <br /> Once we have them all disposed of I will send you copies of the manifest." <br /> 01/11/2018 Emailed Ty to ask about the waste disposal records. Ty replied "Bins are all PE 2546 SAEED <br /> 6 min disposed of, Probably be in the office on Monday and I can send over the SC 544 <br /> manifest copies" <br /> 01117/2018 Emailed Ty and Ash Transport asking for disposal records. Two non-haz PE 2546 SAEED <br /> 24 min manifests were emailed to me by Jo Ann Davis with Patriot. I reviewed the SC 544 <br /> manifest, and reply the email that are two manifest missing. Jo Ann sent me the <br /> other manifests and she stated "Attached is the missing 2 manifests, sorry about <br /> that guess they did not make it through on the scan. I have not received the <br /> waste analysis but I have a request out to our chemist Raj for the report. As soon <br /> as I receive I'll send your way. " <br /> I reviewed and printed the manifests with the emails and attached to the report. <br /> Page 3 of 4 <br />
