PHSIEHD has a contract with California State Water <br /> Resources Control Board to conduct an underground storage tank corrective action program. The <br /> above referenced site was placed in the PHSIEHD Local Oversigl t Program (LOP) in June 1996 due <br /> to an unauthorized release that was discovered during UST removal in September 1985. <br /> In a letter dated March 7, 1994 (see enclosure), PHSIEHD informed you that an investigation to <br /> determine the full extent of the soil and groundwater contamination at the above referenced site was <br /> necessary. To date, no investigation has been performed. <br /> Please submit a work plan to define the lateral and vertical extent`of contamination at this site that <br /> includes a groundwater investigation. The work plan, PHSIEHD permit application, and $89 permit fee <br /> should be submitted to PHSIEHD by May 31, 2000.
