Box 944212 O <br /> Sacramento, CA 94244-2120 <br /> APR 191995 <br /> Subject: Claim No. 008502 <br /> Weldon Church Property ENVIRONMENTAL HEALTH <br /> 104 West Beverly, Tracy, California PERMIT/SERVICES <br /> Dear Ms. Bracco: <br /> In a letter date 29 March 1995, the Underground Storage Tank Cleanup Fund (USTCF) <br /> notified Mr. Weldon Church that the referenced site is no longer eligible for reimbursement. <br /> The proposed withdrawal of the claim is based upon lack of compliance with corrective <br /> action directives issued by the San Joaquin County Public Health Services - Environmental <br /> Health Division (PHS/EHD). <br /> On 15 February 1995, a work plan for excavation and installation of groundwater monitoring <br /> wells at the site was completed by Geological Audit Services, Inc.; a copy of the work plan <br /> is enclosed. Subsequently, the work plan was approved by Mr. Ron Rowe of the PHS/EHD. <br /> However, the PHS/EHD has stated that prior to excavation at the bite, the proposed work <br /> must undergo a period of public participation. We anticipate the public participation <br /> process to commence on 17 April 1995. Upon completion of the required public <br /> participation period, AGE will immediately begin excavation of the former UST area, in <br /> accordance with the work plan. After excavation, monitoring wells will be installed. <br /> Therefore, on behalf of Mr. Weldon Church, we request a review and, if necessary, <br /> reinstatement of USTCF Claim No. 008502.
