The work outlined M <br /> in this plan has not occurred. The work as outlined in this approved Work Plan must be <br /> completed by February 29, 2000. If the work is not completed within that time frame the <br /> site will not be in compliance with PHS/EHD directives and reimbursement for corrective <br /> action costs from the State Water Resources Control Board Cleanup Fund may be <br /> jeopardized. <br /> A well permit application and $89.00 permit fee should be submitted to PHS/EHD as <br /> soon as possible. After the permit has been issued the work can be scheduled.
