Environmental Health Department <br /> N <br /> Complaint Record Daily Time and Activity Summary from 3/3/2012 to <br /> 4(�FOFN� 10/30/2016 for CO0042459 located at <br /> 4075 E MAIN ST <br /> Activity Date Recorded by <br /> 10/25/2016 5:30-7:OOpm Received page from SJGH regarding a 2-3gallon gasoline spill. PE 2546 HENRY <br /> 90 min Called reporting party, employee Anthony. He explained he had used some SC 904 <br /> absorbent and cleaned up the spill. He put the absorbent in a 5 gallon bucket. <br /> Went to site. Met with Anthony and he showed me how he cleaned up the release <br /> and where the waste was stored. Spoke with Jason Steele and he explained he <br /> would be taking the hw to the HHWF when he had an appt. Thanked them for <br /> their time and left. <br /> 10/26/2016 Discussed with LT. We decided not to charge for this complaint based on the RP PE 2546 HENRY <br /> 60 min cleaned up the release appropriately. Sent email to Jason Steele and let him SC 004 <br /> know. Printed pictures and abated complaint. <br /> Page 1 of 1 <br />
