Pacific Ave. CITY: Lodi I ZIP CODE: 95242 <br /> OWNERIOPERATOR: Mitch Slater, Director of Maintenance& Operations TELEPHONE: (209) 331-7181 <br /> TYPE OF M WARNING CENTER RESPONSE ❑OTHER PROGRAM 2546 RECORD 000045285 <br /> INSPECTION: U COMPLAINT❑CONSULTATION ❑ ROUTINE ELEMENT: ID#: <br /> NATURE OF WASTE DISCHARGE/SPILL RELEASE: The custodial storage Container caught fire, causing a release <br /> of chemicals. <br /> OBSERVATIONS: Liquid from fire clean up was observed on the ground amongst the burned waste. <br /> Some liquid spread to the soccer field behind the container. A small amount of liquid entered the <br /> storm drain just north of the container, but did not appear to spread to adjacent storm drains. Ramos <br /> Environmental is on site and handling the clean up of the waste, container, and storm drain. Samples <br /> are being planned for the possible contamination of the soccer field. <br /> VIOLATIONS: <br /> ® Discharge of any Waste, including Sewage <br /> (CA Health and Safety Code 5411) <br /> ❑ Discharge of Waste or Sewage to Water or within 150' of Waterway <br /> (SJC Ordinance Code Sections 9-1125.3, 9-1125.4) <br /> ® Improper Disposal of a Hazardous Waste <br /> (CA Health and Safety Code 25189.5) <br /> ❑ Permitting or Maintaining a Nuisance <br /> (SJC Ordinance Code Section 8-5100) <br /> ❑ Other: <br /> CORRECTIVE ACTIONS/ORDER: <br /> Immediately contain, clean up, and dispose of the waste at an authorized, permitted location. <br /> Ensure that clean up personnel possess adequate training. <br /> P9 Investigate and abate contamination under the oversight of the appropriate lead agency. <br /> 159 Submit copies of hazardous waste manifests/disposal records to the EHD within 30 days. <br /> ❑ Other: Correct By: <br /> COST RECOVERY:All EHD Staff Time Associated with this Spill Response is Billed to the Responsible Party at <br /> the CurrenlHourlir y Rate.
