Patterson Pass Rd CITY:Tracy I ZIP CODE: 95377 <br /> OWNERIOPERATOR: Shell Pipeline I TELEPHONE: 661-391-2429 <br /> TYPE OF El WARNING CENTER RESPONSE ❑OTHER PROGRAM RECORD <br /> INSPECTION: ®COMPLAINT❑CONSULTATION ❑ ROUTINE ELEMENT: 2546 ID#: CO <br /> NATURE OF WASTE DISCHARGEISPILL RELEASE: Approximately 50 gallons Of Crude oil was released from two oil transfer <br /> pumps after pump(s) malfunction and the subsequent fire. <br /> OBSERVATIONS: Due to facility's requirement for a specialized PPE (fire retardant gear) to enter the spill area, no close <br /> inspection and clean-up verification could be conducted by the EHD on May 25, 2015. Please be aware that all impacted <br /> soil has to be excavated and disposed off under the appropriate waste manifesting procedure[HSC 25160 (b)(3)]. The <br /> disposal of any hazardous waste, or the causing thereof, is prohibited when the disposal is at a facility which is not permit- <br /> ted or authorized for hazardous waste disposal. [HSC 25189.5 (a).] Notify the EHD in advance of all follow up clean up <br /> actions, (tentatively, further clean-up is scheduled for May 26, 2015). <br /> VIOLATIONS: <br /> ❑ Discharge of any Waste, including Sewage <br /> (CA Health and Safety Code 5411) <br /> ❑ Discharge of Waste or Sewage to Water or within 150' of Waterway <br /> (SJC Ordinance Code Section 9-1125.3, 9-1125.4) <br /> N Improper Disposal of a Hazardous Waste <br /> (CA Health and Safety Code 25189.5) <br /> ❑ Other: <br /> CORRECTIVE ACTIONSIORDER: <br /> Immediately contain, clean up, and dispose of the waste at an authorized, permitted location. <br /> Ensure that clean up personnel possess adequate training. <br /> ® Investigate and abate contamination under the oversight of the appropriate lead agency. <br /> ® Submit copies of hazardous waste manifests/disposal records to the EHD within 30 days. <br /> ❑ Other: Correct By: Immediately <br /> COST RECOVERY: All EHD Staff Time Associated with this Spill Response is Billed to the Responsible Party at <br /> the Current Hourly Rate.
