PE 2546 ZAREEF <br /> Called Thelma Sass th a Specialist, Environmental Safety& Health personnel. SC 544 <br /> She confirmed that as a pallet of bleach was being removed from the rack, the <br /> pallet disintegrated and 20 gallons of bleach spilled on the floor. She said that <br /> absorbent was placed on the bleach. <br /> 1:30 pm arrived on site. Spoke to Thelma Sass. Went into warehouse in aisle 49 <br /> where spill occurred. There was bleach on the floor and empty containers of <br /> bleach. Absorbent was placed on the spill. Thelma said that a third party clean up <br /> company would arrive to clean up the spill. The facility employees were told to stay <br /> out of the spill area. There were no injuries or drinking water affected. No open <br /> drains inside the facility. Issued Noticed to Abate to Thelma Sass. <br /> 05/23/2016 Filled out Prop 65 gave to Garrett Backus for apporval. PE 2546 ZAREEF <br /> SC 203 <br /> 05/23/2016 At 1205 1 received a call from Desiri Sadra to let me know that Thelma Sass from PE 2546 SAEED <br /> United Grocers in stockton called to notified us of sill at the United Grocer SC 004 <br /> Warehouse located on 1990 N Picoli road in Stockton. The spill occurred at 1145. <br /> A pallet of Bleach was dropped into warehouse floor and 20 gallons of Bleach <br /> were spilled. No waterways or drains were impacted. NRC was notified of the <br /> incident and a third party contractor on their way to clean up the Spill. AT 12:16 1 <br /> paged the EHD officer and Aris Veloso returned the call and I notified the incident. <br /> Aris told me he is at another incident and he has another incident to respond to in <br /> 45 mintures, and I should call Fatinah Zareef. At 1218 1 called Fatinah Zareef and <br /> notified her of the incident. <br /> 05/23/2016 See ER report PE 2546 HENRY <br /> 944 <br /> 05/24/2016 1 entered the narrative for this incident <br /> PEE 2546 SAEED <br /> 05/24/2016 nSC 004 <br /> was cleaned to EHD satisfaction.
