Matt Short to respond to N West Lane, PE 2548 LOPEZ <br /> 180 min between Morada Lane and Eight Mile Rd., on the west side, for an abandoned fuel SC 904 <br /> tank with fuel; sent text to Garrett for approval, waited 5 min. for a response; sent <br /> text to Muni for approval; Muni called and approved response; 1515–arrived on <br /> scene; SFD Capt. Matt Short provided a copy of the Site Safety and Control Plan <br /> and advised SFD's Haz Mat ID result indicated kerosene; transferred <br /> approximately 15 gallons of fuel from the tank to into a 55 gallon drum; loaded <br /> drum and fuel tank for transport to the HHW; 1605–depart scene; arrive at HHW, <br /> off load drum, complete the Public Works Waste Pickup and Transportation <br /> documentation, made a copy of the Waste Pickup documentation; restock HM2 <br /> with a 55 gallon drum; depart HHW; 1745–arrive at home. <br /> 05/30/2017 Completed incident narrative; added text to photos, printed a overhead site map PE 2548 LOPEZ <br /> 120 min from parcel view; advised Jamie that I used the last disposable pump on HM-2; SC 007 <br /> contacted 011ie at the HHW regarding the empty fuel tank; 011ie advised the HHW <br /> does not accept fuel tanks; Discussed the status of the fuel tank with Garrett and <br /> Muni; Muni advised he will be working with Public Works to have the tank removed <br /> from the HHW. <br /> 07/12/2017 Generated a LOG 2 report; printed a 5104; added date the photos were taken and PE 2548 LOPEZ <br /> 42 min reprinted photos; forwarded complaint package to Garrett for review; later, SC 007 <br /> corrected the service code from 004 to 007 and reprinted the LOG 2 per Garrett. <br /> Page 1 of 1 <br />
