COMPLAINANT CANCELED THEIR FOOD ORDER AND WALKED OUT. <br /> i <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as'MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br /> the food facility. A <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION --TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Observed one live cockroach in drawer of prep table in kitchen; kitchen employee attempted to catch and remove but may <br /> have been unsuccessful. Spoke to another employee who stated they are aware of cockroach problem. Last time Clarke <br /> came out was around Thursday. <br /> Observed food debris on floor and shelves at and behind cook range, prep table, reach in and equipment. Clean and remove <br /> food source to vermins. <br /> Observed leaking pipes under spray sink and warewash machine. Repair and prevent wet floor and wet space which can <br /> attract cockroaches. <br /> Contact pest control contractor for additional effort. <br /> Time for this inspection is billable.
