MONITORING AND REPORTING PROGRAM ORDER NO. R5-2008-0149-XXX 3 <br /> STALLWORTH AUTO SALES <br /> SAN JOAQUIN COUNTY <br /> provided in those technical or monitoring program reports, is guilty of a misdemeanor, may be <br /> civilly liable in accordance with subdivision (d)... <br /> (d)(1) Civil liability may be administratively imposed by a regional board in accordance with <br /> Article 2.5 (commencing with Section 13323) of Chapter 5 for a violation of subdivision (c) in an <br /> amount which shall not exceed five thousand dollars ($5,000) for each day in which the violation <br /> occurs. <br /> It is Hereby Ordered that the Discharger shall comply with the following Monitoring and <br /> Reporting Program (MRP) requirements: <br /> GENERAL REQUIREMENTS <br /> 1. All samples should be representative of the volume and nature of the discharge or matrix of <br /> material sampled. The time, date, and location of each grab sample shall be recorded on <br /> the legal chain of custody form. <br /> GROUNDWATER MONITORING <br /> 2. As shown on the attached Figure 2, there are fourteen (14) existing groundwater monitoring <br /> wells (MW-1A, MW-113, MW-1C, MW-1D and MW-2 through MW-11) at the site. The <br /> groundwater monitoring and reporting program (MRP) for the groundwater monitoring wells <br /> and any groundwater monitoring wells installed subsequent to the issuance of this MRP, <br /> shall follow the schedule below. Groundwater monitoring wells with free-phase petroleum <br /> product or visible sheen shall be monitored, at a minimum, for product thickness and depth- <br /> to-water (DTW). Sample collection and analysis shall follow standard Environmental <br /> Protection Agency (EPA) protocol. <br /> For the purposes of the proposed ISCO injection study, the wells have been divided and <br /> identified as background wells (MW-2 and MW-9), treatment zone wells (also referred to as <br /> performance indicator wells; MW-1A, MW-8), transition zone wells (MW-6 and MW-7), and <br /> compliance zone wells (MW-10). All other wells are either cross-gradient or are discretely- <br /> screened wells monitoring water quality and occurrence in deeper portions of the saturated <br /> zone beneath the site; therefore, these wells will not be used as an ISCO injection study <br /> observation point, but will continue to be monitored/sampled as part of the ongoing semi- <br /> annual groundwater monitoring program. <br /> The groundwater monitoring wells shall be sampled according to the schedule in Tables 1A <br /> and 1 B and the samples analyzed by the methods in Table 2, as follows: <br /> Table 1A: Samplings Frequency (During ISCO Pilot-Scale Testing) <br /> Well ID Frequency Constituent Suite(s) Monitoring Objective <br /> MW-1A,MW-8 Other8 Suites A and B Treatment Zones <br /> MW-6, MW-7 Other" Suites A and B Transition Zone <br /> Well IDs shown on attached Figure 2 <br /> 2 i.e.,weekly, monthly,quarterly,annually, other <br />
