The work as approved by PHS/EHD in the <br /> Preliminary Site Assessment Work Plan dated December 4, 1998 prepared by Advanced <br /> GeoEnvironmental, Inc., has not been completed. <br /> There was an additional unauthorized petroleum release'at this site on January 29, 1999. <br /> This additional contamination was not addressed in the above referenced Work Plan. <br /> Changes in site conditions may have occurred at this site since'the above Work Plan was <br /> approved. For these reasons it will be necessary to submit a new Work Plan. A new <br /> Work Plan to determine the lateral and vertical extent of contamination at this site should <br /> be submitted to PHS/EHD by December 10, 1999.
