Investigative work was conducted at the site on September 22, 1998 and <br /> January 28, 1999 as per the above referenced Work Plan On January 28, 1999 during <br /> the course of borehole construction a product line was ruptured by the drill bit causing a <br /> petroleum release to the subsurface soils at the site. On,jFebruary 1, 1999 soil samples <br /> were collected in the area of the product line release. The work as outlined in the Work <br /> Plan has not been completed and PHS/EHD has not received a�report of findings for work <br /> that occurred at the site on the dates referenced above. <br /> i <br /> Complete the work as outlined in the above referenced Work Plan by July 30, 1999. A <br /> report of findings (including evaluation of the petroleum release on January 28, 1999) <br /> should be submitted to PHS/EHD within 60 days of on site work completion. Any <br /> modifications to the originally approved Work Plan must be approved of by PHS/EHD <br /> prior to work start. Notify PHS/EHD 48 hours in advance of any planned work activities <br /> at the site.
